How do cats know when/where to clean themselves?

I've had a cat for 8+ years and am simply curious how she decides what part of her body to clean and when. Does she think something is dirty? Does she have a schedule I don't notice? Mostly her walnut-brained mind seems to work randomly, but cats are usually good at keeping themselves clean so it must work.

    Purely by smell and feel of matted/dirty hair.

    "This smells bad" or "this feels good" or "I itch." Mostly the second two. The "smells bad" one seems to mean Come Shove Your Ass Into Your Human's Face.
