Where do I file a car accident report in Seattle?

I can't find it anywhere. I don't need a copy of a report, I need to file a report of an accident.

You can report an accident yourself as long as it is for damages below $700 and where no personal injuries occurred to any party involved. I believe both parties need to file a report.

Here are both empowered and non-empowered PDF forms:

It depends on what you're trying to do: you either report to the police, or to your insurance company. (Let me know if you've got something else in mind). Not sure if the police will take a report after the fact -- generally if you want to file a police report you should call 911 from the scene and have an officer take the report at the scene.
